Notice of Privacy Practices

This notice describes how medical information about patients of the Nirvanix Health Austin clinic may be used and disclosed and how patients can access that information. Nirvanix's national telehealth program uses Scriptful infrastructure; the medical group responsible for an available service provides its own notice before clinical intake.
Our responsibilities
We are required by law to maintain the privacy and security of protected health information, give you this notice, follow the notice currently in effect, and notify affected individuals following a breach of unsecured protected health information when required.
How we may use and disclose information
We may use or disclose protected health information for treatment, payment, and healthcare operations; to people involved in your care when permitted; for public health and safety activities; for health oversight, workers' compensation, law enforcement, organ donation, research, or legal proceedings when applicable law permits or requires it. Other uses, including most uses for marketing or the sale of protected health information, require written authorization unless an exception applies. You may revoke an authorization in writing, except to the extent already relied upon.
Your rights
- Inspect or obtain an electronic or paper copy of your record.
- Ask us to correct information you believe is incorrect.
- Request confidential communications or certain restrictions.
- Obtain an accounting of certain disclosures.
- Receive a paper copy of this notice at any time.
- Choose someone with legal authority to act on your behalf and exercise these rights.
We may charge a reasonable, cost-based fee where permitted. We will explain in writing if a request cannot be granted.

Changes to this notice
We may change this notice and apply the revised notice to information already maintained. The current version will be available at the clinic and on this website.
